WHAT IS A BASIC INFERTILITY STUDY?
The basic infertility study evaluates ovarian function, follicular reserve, and the health of the woman’s reproductive organs, including the uterus and fallopian tubes. For men, after a detailed medical history is taken, a blood test and a comprehensive semen analysis are performed to assess sperm quantity, quality, and motility.
It is crucial for both partners to provide the specialist with detailed information about their medical history, including any past or present illnesses, infections, medications, and lifestyle habits. A thorough medical history can reveal potential issues that may be contributing to infertility. After a general and gynecological examination, the specialist will determine which tests should be included in your basic infertility study.
WHEN SHOULD A BASIC INFERTILITY STUDY BE CONDUCTED?
A basic infertility study should be initiated if pregnancy has not been achieved after one year of regular, unprotected sexual intercourse in women under 35 without a history of fertility-related issues, or after six months in women aged 35 and older. In certain cases, earlier intervention may be warranted if circumstances suggest the need for an earlier evaluation.
It is important to consider the woman’s age, as reproductive capacity begins to decline after age 35, with a more significant drop after age 40.
WHAT TESTS ARE INCLUDED IN A BASIC INFERTILITY STUDY?
First, it is essential to assess the personal and family medical histories of both partners. Lifestyle factors that could negatively impact pregnancy success, such as the use of toxic substances, drug use, or poor dietary habits, should also be reviewed.
While the basic infertility study has been greatly simplified over the years, there are still a few essential tests for both men and women. If any results indicate an issue, specific treatments may be recommended. These key tests include:
SEMEN ANALYSIS
A semen analysis is essential for evaluating the male factor in a couple’s infertility. This test assesses various parameters of the seminal fluid, including sperm count, motility, vitality, and morphology.
HORMONAL TESTING
Ovarian reserve is assessed by measuring anti-Müllerian hormone (AMH) levels. This marker helps predict ovarian response to stimulation, particularly in the context of assisted reproduction. Progesterone levels in the second half of the menstrual cycle indicate whether ovulation has occurred. In women with irregular or absent menstrual cycles, more comprehensive hormonal testing may be required, including assessments of androgens, prolactin, thyroid hormones, and more.
TUBAL PATENCY ASSESSMENT
Determining whether the fallopian tubes are unobstructed is crucial, especially if artificial insemination is being considered. This can be assessed through hysterosalpingography (HSG) or hysterosonography, which avoids the use of contrast dye and exposure to radiation. The use of 3D technology can also help detect small uterine malformations or anomalies, completing the evaluation of the uterine cavity.
HIGH RESOLUTION TRANSVAGINAL ULTRASOUND
Transvaginal ultrasound provides information about the uterine cavity, ovaries, and fallopian tubes. Antral follicle count is also important as another marker of ovarian reserve.

STUDY AND DIAGNOSIS OF FERTILITY

WHAT HAPPENS AFTER A DIAGNOSIS OF INFERTILITY?
The basic infertility study may be sufficient to identify the cause(s) of infertility, allowing us to initiate the appropriate treatment. However, additional investigations are often necessary depending on the results of the basic study. This pre-treatment process usually takes very little time—typically no more than two weeks from the initial consultation to receiving the results.
In some cases, all test results may appear normal. This occurs in 20% to 30% of couples, leading to a diagnosis of unexplained infertility. While this doesn’t mean there isn’t a problem, it suggests that the issue hasn’t yet been identified and may require more advanced diagnostic techniques, including testing in the IVF lab to uncover potential issues related to egg quality, fertilization, or embryo development.

In nearly all cases, fertility treatment can begin immediately, starting with the woman’s next menstrual cycle if desired. According to the law, serological tests are required before starting the treatment to rule out infectious diseases, in accordance with the regulations of each country.
